Student Contact InformationAt the beginning of the year, parents fill out the sheet shown above. Within the first week of school, I place these sheets and a parent contact log into a binder with numbered tabs for each student. Whenever I need to make a parent phone call or other contact, I can pull out the information sheet and record a brief overview of our discussion.
*Personal images included Guided Reading RecordsWhen keeping records for guided reading, it is best that individual teachers develop their own systems that work the best for their classrooms. Pinterest and Teachers Pay Teachers can provide great resources for creating the system that works best for each teacher. Linked above are a few of the blogs that I used to create the most effective system for my classroom. I keep resources in 1 binder behind my guided reading table and student notes and anecdotal records in another binder. Keeping records in this way is great for conferences and using anecdotal data to fill out report cards.
