Anecdotal NotesAt the beginning of the year, I set up a Google Document for each student. When I see ongoing behaviors or areas of concern, I type in the date, the behavior, and any intervention I have applied for the student. This is also true if I begin an intervention in reading or math. This anecdotal document serves a purpose during student intervention/support/IEP meetings, when filling out report cards, when deciding which intervention to try next, or when talking to next year's teacher. I utilize this document as an anecdotal overview of the student. Not all students will have information in this document.
In my district when students receive specific interventions or have major behavior concerns, we use another tracking system as well that can only be accessed while at school. I copy this information into that document as necessary. *Personal images included |
Skill/Growth ChartsIn order to track major student progress and see growth/where growth is not happening, I create a few color coded charts to visually analyze student data. The two types of data that I currently have charts set up for are word features data (our PLC goal) and reading data (guided reading level and sight word numbers). Shown above are samples (not actual class data) of our word features scores from unit 1 to unit 3. Student names would be listed to the left.
