This resource follows a similar structure to the Danielson's Framework model with a different breakdown method of proficient actions and examples.
Scholastic Record Keeping and Recording
Scholastic's article for teachers gives additional examples on how to keep and create portfolios on students. The article delves into methods of creating scoring rubrics as well.
School Talk - Record Keeping
The School Talk article is an older resource for record keeping but many concepts still ring true. Included is a list of work samples that teachers may want to keep in order to build a record about each student. In the second half of the article, Kidwatching is discussed as the article describes how to keep written and observational notes about students.
*Changes could be made to these ideas for the digital age, but a strong list of what items to keep records of is included.
